05/13/2010 - Rescue Box


On Thursday evening, May 13, 2010 St Leonard and PFVRS were dispatched to motor vehicle
collision on Mackall Rd at Collins Court.  Squad 7, Ambulance 78, Ambulance 79, Engine 71 and
Ambulance 48 immediately responded with full crews (with additional staff at SLVFD).

En-route, Dispatch advised the police officers on scene reported the MVC as serious and Squad
2 was added to the box.  Upon arrival of A-79 EMS Sgt. Hall provided BIR of two serious patients:
Trauma Category B and C and a helicopter was requested.  Squad 7 arrived immediately after the
ambulance, Chief 7B established command giving the technical extrication of the car to Squad 7,
the SUV extrication to Engine 71 and the landing zone to Squad 2 and Chief  2A.  And additional
ambulance was requested and filled by PFVRS. A second helicopter was requested when the
Trauma Classification of the victims became A & B and the Park Police helicopter was dispatched
to support Trooper 7.  Squad 7’s crew under the direction of Rescue Lieutenant Anderson with
Sergeant Bowen, Firefighter Pavis and Driver/Operator Rollyson began the complicated
extrication of the victims of the car while Engine 71’s crew under the direction of Safety Officer
Lankford with Firefighter James, Firefighter Whitall, Firefighter Santivasci and Driver/Operator
Baker  quickly extricated the two passengers of the SUV with the ambulance crews from SLVFD
and PFVRS.  The SUV passengers were both taken to Calvert Memorial Hospital.

Engine 71’s crew then supported Squad 7 along with Chief 2B.  The victims were interdependent
(any movement on the driver’s side impacted the passenger); both being pinned by the
dash/engine with the passenger additionally pinned by the axel and non-intact floor board. A
technical extrication with multiple tools and equipment separated the car from the passenger and
then the passenger was quickly removed to the waiting flight medic (who was brought to the
scene) and SLVFD ambulance crew. Immediately after the passenger was removed a quick
completion of dash roll and cut of petals enabled the removal of the driver to the waiting CALS
medic and PFVFD ambulance crew.

During the extrication Dispatch informed Command that the second helicopter was turned around
due to weather and Trooper 7 may be grounded.  It was decided by the medics to ground
transport both patients to PG Trauma. The flight medic then performed RSI of the passenger on
scene.  However during transport they diverted to CMH where the patient was stabilized and
successfully transported by MedStar to Washington Hospital Center.  The driver was transported
to CMH by PFVRS ambulance crew and CALS PMIC Fenwick to PG Trauma.  Squad 7 remained
on scene to support the police with accident re-construction.
